Summary of Function:

The Educational Assistant – Student Medical Support provides classroom‑based educational support to a student with special learning and medical needs. The position supports the student’s academic progress, behaviour, and social development under the direction of the classroom teacher and Student Services.

In addition to core educational assistant duties, the position includes specialized responsibility for supporting the safe day‑to‑day management of Type 1 diabetes in accordance with the student’s individualized Health Care Plan, in collaboration with the school nurse, educators, and parents/guardians.

Job Duties:

  • Supports the student’s academic progress, behaviour, and social development while working within the classroom management structure, discipline plan, and expectations outlined by the teacher and Student Service Department

Specialized Responsibilities – Student Medical Support

  • Assists in monitoring the student’s diabetes throughout the school day in accordance with the student’s individualized medical care plan.
  • Supports blood glucose monitoring and recognizes signs and symptoms of hypoglycemia and hyperglycemia.
  • Assists the student with insulin administration as directed in the medical plan and under the guidance of the school nurse and other trained professionals.
  • Supports carbohydrate counting and meal/snack monitoring to help ensure appropriate insulin calculations and safety of the student
  • Performs all medical-related support duties in accordance with the student’s medical care plan, school protocols, and guidance from the school nurse or designated health professionals.
  • Responds appropriately in the event of a diabetes-related medical emergency and follows established emergency procedures, including notifying parents/guardians, administration, the school nurse, and emergency services if required.
  • Willing to participate in training sessions with the Maria Hospital Diabetes Management Team
  • Maintains accurate daily records related to glucose monitoring, insulin administration, food intake, and any medical concerns.
  • Weekly reporting to medical team
  • Communicates relevant information that may impact the health, learning, or behaviour, to the appropriate resource teacher, Special Education Coordinator, and school nurse, parents/guardians as required.
  • Ensures the student has appropriate supervision during transitions, recess, lunch, field trips, and extracurricular activities where medical monitoring is required.
  • Supports the student’s independence and self-management skills over time, as appropriate to age and ability.

Requirements:

  • Knowledge of Mi’gmaq history, culture, and traditions
  • A High School diploma from a recognized institution or equivalent.
  • Training in the area of LPN, PSW, or equivalent post-secondary courses in a related area from a recognized institute and or related experience would be an asset.
  • Ability and willingness to participate in ongoing medical training related to diabetes care and student safety.
  • Experience supporting students with medical needs, particularly diabetes, would be considered an asset
  • Ability to pass a criminal background check.

Summary of Function:

The Personal Care Worker (PCW) contributes an important role within the Waqatasg Nursing Program Team.  They are responsible for aiding with the daily routines to residents along with activities of daily living. The PCW will ensure that all services are provided in a caring and respectful way in keeping with all relevant policies and procedures. Work directly with all staff of the Waqatasg Nursing Program in the delivery of services identified within the client’s individual care plan and work directly with clients and family members to meet the needs in the plan.

 

Job Duties:

  • Perform light housekeeping duties such as tidying residents’ rooms and collecting the garbage, cleaning closets, hanging clothing, making beds, cleaning wash basins, dental care cups, etc.
  • Must be willing to do shift work.
  • Answer call bells and redirect when appropriate.  Provide items of comfort, sitting with residents/clients and having purposeful conversations, etc.
  • Assisting with the residents’ dining experience under the direction of the Nurse, including, but not limited to; Escort residents to and from dining areas and setting up and clearing tables, pouring beverages, and delivering meals to residents.
  • Assist and support resident programming such as but not limited to; Recreational Programs, Visitation Program, Assist in the preparation and cleanup of activities rooms and visiting areas.
  • Assist clients with different outings and activities.
  • Follow medical instructions carefully and consistently.
  • Make recommendations for changes and improvements for the Individual Care Plan as required.
  • Provide information to other health care professionals, when requested.

Requirements:

  • Completed the Institutional and Home Care Assistance Program and/or previous experience in senior care, group homes, caring for a family member/friend is an asset.

Summary of Function:

The function of the Licensed Practical Nurse (LPN) is to effectively deliver client care plans on behalf of LCHS Nursing and Home Care Programs.  This includes services and/or activities in the following health areas: Home Care Program, Community Clinic, Immunization Program, and the Diabetes Program. The LPN is Responsible for providing prevention and treatment services including wound care, medication management, foot care, palliative care, specimen collection, post operative care, etc., under the supervision of the Nurse Supervisor.  Local travel is required.

Job Duties:

  • Utilize their clinical nursing skills to effectively deliver client care plans, i.e. wound management.
  • Monitor the clients’ care to ensure that the care plan is carried out according to the assessment and plan outlined by the Eligibility Review Committee (ERC).
  • Monitor the client’s mental health status, i.e. family dynamics, interpersonal relationships, and report any concerns to the Home Care Nurse.

  • Provide medical services/treatment to community members on a rotational basis within the LCHS Community Clinic.
  • Provide assistance to the Immunization Program and the Diabetes team with the execution of the program activities/requirements.
  • Assist with the delivery of other LCHS programs and activities, i.e. Annual Community Wellness Fair, Flu Clinics, etc.
  • Install bed rails and set up beds in homes of clients where this is a necessity.

Requirements:

  • A License in good standing from the Ordre des Infirmieres et Infirmiers du Quebec (OIIQ).
  • Maintain current CPR Certificate.
  • Maintain Valid Driver’s License.

Purpose of the Position

The Career & Employment Counselor works directly with LMDC clients by providing career counseling, developing individualized action plans, addressing employment barriers, and connecting clients with training, funding, and job opportunities.

Job Description

Under the Supervision of the LMDC Director, the Career & Employment Counselor performs the following duties:

  • Complete necessary employability assessments and needs determination with Listuguj Mi’gmaq and assisting in the development of formal return to work action plans.
  • Provide information, direction, advice, and support for job readiness skills, job search strategies, writing résumés and preparing for job interviews
  • Assess need for assistance such as rehabilitation, financial aid or further training and refer clients to the appropriate services
  • Counsel clients on career exploration and planning
  • Provides information on LMDC’s support measures, professional training and job creation programs
  • Assists client with funding application for programming
  • Provide established workers with information and strategies for maintaining a job, dealing with job dissatisfaction, making mid-career changes and adjusting to workplace transitions
  • Provides information on LMDC’s job creation programs to potential employers/sponsors; and facilitates and promotes community employment initiatives to secure employment opportunities for Listuguj Mi’gmaq.
  • Provide workshop facilitation on these and other work-related topics when needed.
  • Monitors Clients’ participation on training and employment measures.
  • Ensures information is entered into the labour market database
  • Complete reporting requirements

 

Qualifications

  • A bachelor’s degree in counselling, career development, education or social sciences or human resources
  • Minimum of 3 years work experience in counselling or human/social services

Casual Medical Driver

Summary of Function:

The Medical Van Driver will be responsible for the safe delivery of patients to and from health facilities and will ensure that the transportation vans are not being utilized for personal transport. The medical van driver will be responsible for completing a mileage driver’s sheet for each patient/escort and will submit the mileage sheet at the end of each shift for the month-end report. Other responsibilities will include communicating with the dispatch team and manager and this individual must ensure patient confidentiality at all times. Other duties may be assigned as necessary.

Job Duties:

  • Operates a vehicle to transport clients to and from medical appointments in accordance with the schedule developed by the Medical Transportation Coordinator.
  • Provides special attention to clients who, for medical reasons, require assistance from their homes to the vehicle, into the medical facility, and pick up as required.
  • Maintains health and safety standards by disinfecting the interior of the vehicle and cleaning the outside of the vehicle to ensure all lights, logos and license plates are visible.
  • Reports any irregularities in the operation of the vehicle to the Medical Transportation Program Coordinator at the completion of the shift work.
  • Reports any mechanical malfunctions or potentially hazardous conditions with the vehicle to the Medical Transportation Program Coordinator immediately.
  • Prepares driving/client incident reports within seven (7) days of the incident.
  • Prepares/submits attendance records on a monthly basis
  • Submits LCHS Medical Taxi Attestation forms and expense sheets on a weekly basis.

Requirements:

  • Must hold or be capable of obtaining the following license classes through the Province of Quebec: Class 4A; Class 4B; Class 4C License
  • Class 5 License
  • Must hold a current Certificate of Training for Transportation of Dangerous Goods by Road
  • Must hold a valid driver’s license for the last five (5) years without suspension
  • Must not have a criminal driving offence in the last two (2) years
  • Must possess a current First Aid/CPR Certificate
