Summary of Function:

The Outreach Worker will serve as a frontline support to vulnerable community members by delivering direct, mobile assistance throughout the community of Listuguj. With a foundation in healthcare, this individual will work to connect individuals with existing services and programs offered by the Listuguj Mi’gmaq Government (LMG). The Outreach Worker will provide health-informed guidance, support navigation of available resources, and build trusted relationship with community members in need.

Job Duties:

  • Travel throughout the community to engage with and support individuals in need, particularly those who are vulnerable, isolated, or at risk.
  • Provide basic health assessments, wellness checks, and first-level support.
  • Offer culturally sensitive guidance and referrals to LMG programs and services including health, housing, mental wellness, addictions services, elder care, and social supports.
  • Collaborate closely with LMG departments to streamline access to services and ensure wraparound support.
  • Maintain accurate, confidential documentation of visits, referrals, and services provided.
  • Build strong, trusting relationships with community members to encourage engagement and ongoing support.
  • Identify gaps in service delivery or barriers faced by individuals and communicate these to program supervisors.
  • Participate in community events, outreach campaigns, and initiatives that promote wellness and access to services.
  • Provide informal education on wellness topics, harm reduction, chronic illness management, and preventative care.
  • Uphold client confidentiality, cultural sensitivity, and trauma-informed approaches in all interactions

Requirements:

  • Certificate, diploma, or degree in a medical, nursing, or allied health field (e.g., LPN, RN, paramedic, health technician) would be considered an asset.
  • Experience working in Indigenous community health settings is a strong asset.
  • Valid Class 5 driver’s license
  • Strong interpersonal, communication, and problem-solving skills.
  • Knowledge of LMG programs and community resources
  • Ability to work independently and flexibly, including evenings and weekends.
  • Must be able to work flexible hours to meet community needs, with time in-office for documentation and collaboration.
  • Familiarity with Mi’gmaq culture and language is considered an asset.
  • Ability to pass a criminal background/Vulnerable Sector check.
  • In accordance with LMG policy, priority consideration will be given first to qualified Listuguj Mi’gmaq applicants, followed by applicants from other First Nation communuities, followed by other quailifed applicants.
