Summary of Function:

The function of the Program Assistant, Home & Community Care Program is to assist the Manager with the daily functions and delivery of the Home Care Program on behalf of the Listuguj Community Health Services and Health Canada FNIHB. This includes the day-to-day scheduling of Personal Care Workers, attendance at LCHS Programming and Planning Meetings, attendance at meetings relating specifically to the Home Care Program. Travel and training are required.

Job Duties:

  • Assist in the recruitment, orientation and retention of the Home & Community Care Program personnel;
  • Coordinate programs and services directly related to the Home & Community Care Program; (i.e. snow removal; ramps; etc.).
  • Develop, plan and implement daily schedules for Personal Care Workers;
  • Conduct daily staff meetings with Personal Care Workers to discuss up-coming, schedules and concerns of Home Care clients;
  • Act as an advocate for clients, families and staff members of the Home & Community Care Program;
  • Inform the Home Care Nurse of any changes in a clients’ health status as reported either by the client or Personal Care Worker;
  • Develop a communications plan/tools for the program, i.e. flyers and notices;
  • Attend meetings, conferences, seminars and workshops as a representative of the LCHS upon request;
  • Attend the monthly LCHS Staff meeting, Program and Planning meeting and provide a monthly activity report;
  • Maintain and provide statistical information for input into the e-SDRT database;
  • Supervise all Home and Community Care Program contract workers.
  • Ensure monthly attendance reports by Personal Care Workers are submitted in a timely manner.
  • Coordinate the logistics of the LCHS Meals-on-Wheels Program for Home Care Clients, including delivery of meals as scheduled.
  • Assist with the delivery of other LCHS programs and activities, i.e., Annual Community Wellness Fair, Flu Clinics, etc.

Requirements:

  • Ability to effectively communicate in Mi’gmaq is an asset.
  • Strong written and oral communication skills.
  • Strong computer skills, solid knowledge of Microsoft Office Programs;
  • Ability to plan and manage multiple staff schedules using a relevant electronic program;
  • Possess the ability to maintain flexibility / adaptability to meet the needs of the clients, while at the same time administer the program in accordance with Health Canada FNIHB Home & Community Care Program Framework;
  • Knowledge of forecasting expenditures, budgeting and financial reporting and basic accounting principles.
  • Ability to pass a criminal background/Vulnerable Sector check.
