Commercial Fisheries Deckhand Evaluation Tool
This evaluation form is to be completed by the Captain at the end of each fishing season for each Deckhand that has worked on the vessel. Evaluations must be submitted within (30) days of the last fishing trip of the season.
4 Excellent - performance is beyond expectations; 3 Good - performance is satisfactory and consistent with expectations; 2 Fair - performance is consistent with expectations; needs improvement; 1 Poor - performance is below expectations;
